The United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) established by the General Assembly in 1955 assesses the levels and effects of exposure to ionizing radiation on human health and the environment.

This is the first of two volumes of scientific annexes that presents the supporting scientific deliberations for the UNSCEAR 2024 Report to the United Nations General Assembly.

Annex A “Second primary cancer after radiotherapy” evaluates the risk of second primary cancers in patients who have undergone radiotherapy for a first primary cancer. It synthesizes current scientific understanding across oncology, radiobiology, dosimetry, and epidemiology, including a detailed meta-analysis of risks of second primary cancers per radiation dose. The annex also provides insights into biological mechanisms, and implications for public health and radiation protection, underscoring the importance of balancing the benefits of radiotherapy with the long-term risks of second primary cancers.
